Comprehensive Evaluations

To determine the best type of evaluation for your needs, please contact our office. We’ll clarify the purpose of your evaluation, verify insurance coverage (if applicable), and outline next steps for scheduling.

At Autonomy Counseling, PLLC, we provide in-depth evaluations to clarify mental health diagnoses, support treatment planning, and guide personal, academic, or occupational decisions. Each evaluation is individualized, combining clinical interviews, standardized testing, and clear written feedback to help you understand results and next steps.

Insurance Covered Evaluations

Insurance typically covers evaluations when they are clinically indicated for diagnosis or treatment, such as:

  • ADHD and attention concerns

  • Autism spectrum traits

  • Anxiety, depression, or mood disorders

  • Trauma-related symptoms

  • Personality disorder

  • General diagnostic clarification

  • Diagnostic clarification for complex presentations

Please note that we do not conduct educational or academic achievement testing (e.g., math, reading, or dyslexia assessments). For academic testing, we recommend working with a school psychologist or educational specialist.

We Currently Accept

  • Blue Cross Blue Shield (BCBS)

  • HealthPartners

  • Sanford Health Plan

Please note that insurance coverage may vary depending on your specific plan. Prior authorization may be required. We will verify benefits and review any costs before testing begins.

Private-Pay (Non-Covered) Evaluations

Some evaluations are not covered by insurance because they fall outside of clinical or medical necessity guidelines. These include:

  • Court-ordered or forensic evaluations (custody, parenting, risk, etc.)

  • Fitness-for-duty or employment evaluations

  • Evaluations requested by attorneys, employers, or schools without a clinical treatment component

These evaluations are private pay and billed at $250 per hour, in 15-minute increments. Replenishing retainer.

We are NOT currently in-network with the following for testing services: 

  • UCare 

  • Tricare 

  • Optum (United Behavioral Health) 


Clients with these plans may choose to pay privately or request an out-of-network reimbursement form to submit to their insurer.
